Welcome to the ICS Special Scholarship Application page

We have given you several options for applying to this program.

  1. Fill out the email application below, attach requested documents and submit.
  2. Print out the PDF of the application and submit via mail or fax.
  3. Fill out the email form here and fax or email hard copies of your documentation within two weeks of submitting the form.

In addition to your application, please provide the following documentation:

  • Curriculum Vitae
  • Copy of Medical School Diploma
  • Copy of Medical License
  • Specialty Board Certificate
  • Proof of ICS Membership; either member ID number or other proof
All materials must be received no later than June 30, 2006.
